After Hannibal Lecter was caught in 1975, Barney was assigned to be the caregiver for him. Lecter respected Barney for his good treatment and kindly nature he demonstrated as his caregiver for six years. He made appearances in all four books of the series and was portrayed by Frankie Faison in The Silence of the Lambs, the sequel, Hannibal, and the prequel, Red Dragon.

Near the end of the battle, he returned with the dolly when Lecter said, "Barney, take me back to solitary confinement."
